The reason for having two hair transplants

Some are interested in having a hair transplant where they want to fill a large area. The larger the area that you want to cover, the more grafts with groups of hair follicles need to be transplanted. There are particular reasons why we choose to perform two hair transplants instead of one when they need to cover a large area.

For some people who want to get a large area covered with hair transplants, such as the entire scalp, it may be necessary to do it in two procedures instead of one. When we need to cover an area that requires more than 3500-4000 transplants of hair follicle groups, we usually do so over two operations.

The reason we divide a hair transplant into two procedures is due to two factors. One of the reasons we do not transplant many hair follicle groups in the same procedure is that the survival rate of the hair follicle groups is decreasing. This is because hair follicle groups only live for a limited time outside the body. If a surgery takes too long, the risk of some of the hair follicle groups dying is increasing, and thus the result will not be as successful. The dead hair follicle groups cannot produce hair, and thus you will not get a good result. Therefore, we perform it in two procedures if many hair follicles need to be transplanted, to increase the likelihood that all the hair follicle groups we transplant will survive.

The second reason why we divide a hair transplant into two procedures is that there is a risk of so-called overharvesting. This means that you will be picking more hair follicle groups from the donation area, usually from the neck, than you really should. The result of overharvesting is that the neck area will look thin-haired. Picking many hair follicle groups during an operation increases the risk of picking too many. Instead, we choose to split a large hair transplant into two procedures. After the first procedure, it is easier to see how the donation area heals, and thus how many hair follicle groups should be selected in the second procedure, so that it will still look good.
